What do I need to start the pre-hire process?
All applicants will need:
- A completed application, including employment history, identifying info (name, address, etc.)
- Resume
- 2 satisfactory, verifiable written or verbal positive references from a former employer or other person not related to the Applicant
The Wright Agency will complete:
- OIG Fraud Exclusions List Check (LEIE)
- Criminal Background Check from the PA State Police and if applicable, FBI Cogent Fingerprint check (if PA resident for less than 2 yrs.) and ChildLine Verification check
- Face-to-face pre-employment interview with an Agency hiring personnel
What documentation will I have to complete during the on boarding process?
All applicants hired will receive an Offer letter. Each employee will need:
- Verification of training (diploma, transcript, letter from school, attestation)
- Signed 1-9 form (with appropriate identification)
- Copy of state driver’s license
- Copy of auto liability insurance
- Statement of Driving status
- Copy of state license to practice (professionals)
- CPR card copy
- W-4/Social Security card copy
- Availability form
- Copy of current CPR card
- Corporate Compliance Sign off
- Orientation Instruction page
- Conflict of Interest form
- Confidentiality Statement signed
- Signed HIPAA Privacy Statement
- Incident Statement signed
- Signed Do’ s & Don’ t (direct care staff)
- State ‘New Hire’ form
- Proof of Insurance
How many clients/hours will I have?
All staff are hired as “per-diem” status with clients until the employee completes probationary period. After completing the probationary period you will be offered a permanent position.
